Self Declaration Form
This is a self declaration that you will need to give to our R&T every year in April beginning.

Form 49A
As the PAN Card has been mandatory for applying in mutual funds, Form 49A is the PAN number application form. You might need these for your clients.

Registration Form
To access your account online, you will need to fill this form and send it to our R&T. On receiving this, the R&T will assign you a unique User ID and password.

Distributor Nomination Form
You can use this form to nominate the person to whom the commission amount will be payable in the unfortunate event of your death.

Code of Conduct for Intermediaries
It is advised that all intermediaries of the Mutual Fund shall follow the code of conduct.

Option Letter for Transaction Charges
To opt in for transaction charges, kindly fill in the Option letter.
