Additional Purchase/Redemption/Switch/STP Request

If the holder wishes to purchase additional units of the fund he/she has already invested in or he/she wants to redeem the units they hold or would like to switch funds or would like to make a Systematic Transfer (STP) from one scheme to another, click here to download the form. Change of address

If the unit holder wishes to change his/her address in our records, please click here to download the form. If the investment amount was Rs. 50,000 or more, the unit holder will have to get the address changed with CVL (click here to download the form in case of individuals and here in case of non-individuals) and send us the new KYC letter. Change of Bank A/c or mandate

The bank account details with us are used to credit dividends and redemption amounts. If the unit holder would like to change the bank account details, click here to download the form. The request will have to be accompanied with a cancelled Cheque leaf as a proof of the holder’s bank account.

Addition of Joint Holder

If unit holder/s desires to add a second or third holder, then they need to inform us through a letter affixing the signatures of the second/third holder along with the PAN and its proof. Click here to download the draft letter. Please note that if the invested amount is equal to or more than Rs. 50,000 KYC compliance letter of the new holder(s) will also be required. Error in the name of the unit holder

If the unit holder's name has been erroneously recorded due to an error by the unit holder, then to rectify the same the unit holder needs to furnish certain documents to the Fund. If the error has occurred on the part of the Mutual Fund or Registrar the unit holder need not furnish the documents

Requirements:
  • A letter from the unit holder requesting correction of the error. Click here to download the form
  • Certified copy of
    1. Passport/school leaving certificate (in case of an individual);
    2. Certificate of incorporation (in case of a company);
    3. The entry in the Register of Firms (In case of a partnership firm);
    4. Passport/birth certificate (in case of NRIs).

Change of order of Holding

If the unit holder/s desire to interchange the order of holding between them, they need to furnish certain documents to the fund.

Requirements:

Misplaced/Lost Dividends/Redemption Warrants

Unit holders, who ask for fresh dividend/redemption warrant, need to submit certain documents to the fund.

Change of Minority Status of Unit Holder

Unit holder/s who attains 18 years of age need to submit certain documents to the fund.

Requirements:

Change of name

If the change in name is requested by the unit holder the documents mentioned below have to be furnished.

Requirements:
  • Indemnity Bond from the Unit holder on 100 Rupees Stamp Paper. Click here to download the form
  • Any one of the following:
    1. The gazetted notification regarding change of name (in case of individual)
    2. A fresh certificate of incorporation (in case of company)
    3. The entry in the Register of Firms relating to change of name (in case of a registered partnership firm)
    4. Declaration signed by all the partners confirming and declaring the change of name (in case of an unregistered partnership firm). Click here to download the form
    5. Certified copy of passport/documentation under the relevant jurisdiction evidencing the change in name (in case of a NRI/OCB)
  • In case of change of name of a Unit holder due to marriage, the Unit holder would have to furnish the marriage certificate.

Appointment of Guardian

If the guardian is a person appointed by the court or through legal documents the guardian needs to furnish certain documents to the fund.

Requirements:
  • Letter from the Guardian declaring that he/she is the guardian of the minor and stating his/her relationship to the minor along with court order/decree, if any
  • Certified copy of proof of age of the minor (e.g. birth certificate/passport/school leaving certificate etc.)

Pledge of Units

Unit holder/s who wish to pledge the units need to submit the documents mentioned.

Clarifications

  • It is clarified that PAN card copy or another proof of identity of claimant/s is not required separately if KYC acknowledgement issued by CVL is made available
  • KYC acknowledgment address should match with the address in the indemnity bond
  • Where the units are to be transmitted to a claimant who is a minor, various documents like KYC, PAN, Bank details, indemnity should be of the guardian of the nominee
  • With a view to implement common standard documentation, suggested common formats for various documents is attached as annexures for easy reference
  • A ready reckoner matrix of various documents required indicative in nature under different situations is provided for convenience as Annexure VI (click here to download)
  • Indemnity Bond franking amount: Rs. 200
  • Affidavit franking amount : Rs. 100

Transmission to surviving unit holders

Requirements:
  • Letter from surviving unit holders to the Fund / AMC / RTA requesting for transmission of units
  • Death Certificate in original or photocopy duly notarized or attested by gazette officer or a bank manager
  • Bank Account Details of the new first unit holder as per Annexure 1 (click here to download) along with attestation by a bank branch manager or cancelled cheque bearing the account details and account holders name
  • KYC of the surviving unit holders, if not already available

Transmission where nominee is registered

Requirements:
  • Letter from claimant nominee/s to the Fund / AMC / RTA requesting for transmission of units
  • Death Certificate/s in original or photocopy duly notarized or attested by gazette officer or a bank manager
  • Bank Account Details of the new first unit holder as per Annexure 1 (click here to download) along with attestation by a bank branch manager or cancelled cheque bearing the account details and account holders name
  • KYC of the claimant/s
  • Indemnity (franking amount: Rs. 200) duly signed and executed by the nominee/s – Annexure II (click here to download)

Transmission where nominee is not registered

Requirements:
  • Letter from claimant/s to the Fund / AMC / RTA requesting for transmission of units
  • Death Certificate/s in original or photocopy duly notarized or attested by gazette officer or a bank manager
  • Bank Account Details of the new first unit holder as per Annexure 1 (click here to download) along with attestation by a bank branch manager or cancelled cheque bearing the account details and account holders name.
  • KYC of the claimant/s
  • Indemnity Bond (franking amount: Rs. 200) from legal heir/s –Annexure III (click here to download)
  • Individual affidavits from legal heir/s –Annexure IV (click here to download)
  • If the transmission amount is below Rs. One Lakh: any appropriate document evidencing relationship of the claimant/s with the deceased unit holder/s)
  • If the transmission amount is Rs. One Lakh or more: Any one of the documents mentioned below :
    1. Notarised copy of Probated Will, or
    2. Legal Heir Certificate or Succession Certificate or Claimant’s Certificate issued by a competent court, or
    3. Letter of Administration, in case of Intestate Succession

Transmission in case of HUF, due to death of Karta

HUF, being a Hindu Undivided Family, the property of the family is managed by the Karta and HUF does not come to an end in the event of death of the Karta. In such a case, the members of the HUF will appoint the new Karta who needs to submit following documents for transmission:

  • Letter Requesting for change of Karta
  • Death Certificate in original or photocopy duly notarized or attested by gazette officer or a bank manager
  • Duly certified Bank certificate stating that the signature and details of new Karta have been appended in the bank account of the HUF - Annexure I (click here to download)
  • KYC of the new Karta and KYC of HUF, if not already available
  • Indemnity bond (franking amount: Rs. 200) signed by all the surviving coparceners and new Karta - Annexure V (click here to download)
  • In case of no surviving co-parceners OR the transmission amount is Rs. One Lakh or more OR where there is an objection from any surviving members of the HUF, transmission should be effected only on the basis of any of the following mandatory documents:
    1. Notarized copy of Settlement Deed, or
    2. Notarized copy of Deed of Partition, or
    3. Notarized copy of Decree of the relevant competent Court
